1. Weak encoder flange grounding (noise).
2. Phantom alongside the 090/091/092 trio.
3. Failed encoder.

How to fix it

1. Check the trio; reinforce the flange ground.
2. Replace the motor/encoder if it persists; recovery is PULSE RESET then power cycle.
